Canvas May 2014 release & Kaltura CE 3.0.0 integration issue

I was using Canvas December 2013 version and my Kaltura hosted version 3.0.0 was working fine. When I upgraded to Canvas May 2014 release, my Kaltura integration is not working. Canvas Kaltura connection not establishing and my existing videos are not playing. When I tried to upload I am getting the error “Media Comment Application failed to load. Please try again”. Is there any version issue? Can somebody help me in resolving this issue. My production log is listed below:

I am able to connect canvas using Free trial, but when i tried to use the self hosted account i am getting the error “Media Comment Application failed to load. Please try again.”. Please help me resolving this. Lot of existing videos are in my Kaltura instance and I hardly wanted to use in my Canvas application
